Our Mission at 1st Choice Facilitator Service is to maintain your RIGHTS in making decisions about your health and living situation.
We strive to provide support services and consumer-directed programs to individuals and families with the goal to keep clients in their homes and ultimately reduce, if not eliminate, the chances of institutionalization.
Our team of care coordinators and case managers give clients a way to acquire care services through State Waiver programs such as:
- Elderly or Disabled with Consumer Direction Waiver (EDCD)
- Individual and Family Developmental Disabilities Support Waiver (DD)
- Intellectual Disability waiver (ID)
We make use of person-centered practices as we deliver outstanding support for patients and their family. We share what resources we currently have access to and we continually seek other providers within the community that will benefit our clients.
It is our goal to help the individual and the entire family in managing disabilities that limit their physical, mental, developmental and intellectual capacities.
