The Individual and Family Developmental Disabilities Support Waiver (DD) is intended for individuals aged 6 years and above who have been diagnosed with a developmental disability, but not with mental retardation. Such individuals must require care that is of the same level as an intermediate care facility provides.
The DD waiver allows the individual access to services such as:

- Attendant Services
- Companion Services
- Crisis Supervision
- Day Support
- Environmental Modifications
- Family and Caregiver Training
- In-Home Residential Support
- Personal Care Services
- Personal Emergency Response System (PERS)
- Prevocational Services
- Respite Care
- Skilled Nursing Services
- Supported Employment
- Therapeutic Consultation

Our staff will help you file your request for screening with the Virginia Department of Medical Assistance Services. We will also closely assist you in successfully acquiring health and home care services for yourself or a family member.
*1st Choice Facilitator Service provides patients with Personal Emergency Response Systems (PERS). PERS is a medical alert system that enables our patient to obtain emergency assistance when the device’s button is pressed.
