1st Choice Facilitator Service offers assistance for families and/or individuals who are eligible for Medicaid Consumer-Directed services. We allow individuals with disabilities to choose their own attendant – whether a privately hired professional, a friend or a relative – that they can trust to care for them in their homes.
Our function is to help you find qualified attendant(s) and provide assistance in the process of hiring said attendant to take charge of your home care needs. For qualified clients, Medicaid will pay for home care / health services through Public Partnership (PPL). 1st Choice Facilitator Service will facilitate the necessary filing and paperwork for the Consumer-Directed service that is most fitting for your situation.
