The Mental Retardation/Intellectual Disability (MR/ID) Waiver allows the eligible individual to access services such as:
- Assistive Technology
- Companion Services
- Crisis Stabilization
- Day Support
- Environmental Modifications
- Personal Assistance
- Personal Emergency Response Systems (PERS)
- Prevocational Services
- Residential Support
- Respite for Family Members
- Skilled Nursing
- Supported Employment
- Therapeutic Consultation
Eligibility Provisions for the Individual to Receive Care through the ID Waiver
- Must be age 6 or older
- Diagnosed with Mental Retardation
- If under the age of 6, must be at developmental risk for Mental Retardation
- Must be eligible for placement in an intermediate care facility
Types of Waiting List under the ID Waiver:
- Urgent Waiting List – needing waiver services immediately
- Non-urgent Waiting List – needing waiver services within 30 days
- Planning List – will need waiver services at some point in the future
To apply for this waiver, please contact 1st Choice Facilitator Service at 434-444-3502. Our staff will help you file your request for screening with the Virginia Department of Medical Assistance Services. We will also closely assist you in successfully acquiring health and home care services for yourself or a family member.
*1st Choice Facilitator Service provides patients with Personal Emergency Response Systems (PERS). PERS is a medical alert system that enables our patient to obtain emergency assistance when the device’s button is pressed.
